A huge error was made in regards to information entered on death certificate. Wife who completed all documents for funeral home received a death certificate stating her husband did not have a spouse. This certificate took over a month to come. When questioned why this government document had wrong information entered. The funeral home was unsure of how it happened and only stated sometimes errors occur. Funeral home stated that correct information would be resubmitted to vital records. A week had past and decision made to go into vital records. Vital records stated that had not received any updated information from funeral home. Funeral home called numerous times with no response. Finally after getting in contact with funeral home. Updated information was sent to vital records. After waiting over a month spouse lost health insurance due to not having death certificate in timely matter. This is unacceptable.
